PASSPORT
What is PASSPORT?
PASSPORT, as part of a state-wide elder care initiative, is designed to link frail, homebound seniors with available
options for long-term care services. The PASSPORT program is funded and operated by the Ohio Department of Aging
and Ohio Department of Job and Family Services, through the Area Office on Aging of Northwestern Ohio, Inc. The
PASSPORT Program is a department of the Area Office on Aging of Northwestern Ohio, Inc., and is dedicated to serving
all seniors residing in a ten county area.
How does it work?
PASSPORT Case Management staff, Registered Nurses and Licensed Social Workers, provide individuals - in the privacy
of their own homes - with information about community-based services, nursing home placement and the PASSPORT Home
Care Program.
Seniors who are eligible for the PASSPORT Home Care Program may receive in-home services to meet specific needs.
Who is eligible?
PASSPORT Home Care applicants must:
- Be at least 60 years of age
- Permanently reside in a non-institutional setting
- Be financially eligible under Medicaid waiver guidelines -
- Income (under January 2007 guidelines):
- monthly income of applicant above $1215 possible co-pay for services
- monthly income of applicant $1215 or less no charge for services
- spouse's income is also factored to determine payment regardless of applicant's income
- Assets:
- applicant who is single or widow(er) - $1,500
- applicant whose spouse is already on Medicaid $1,500 each = $3,000 total
- applicant whose spouse is not on nor applying for Medicaid - $1,500 applicant $20,328 spouse = $21,828 (minimum standards)
- Require hands-on assistance with at least two of the following: bathing, grooming, dressing, eating/feeding, mobility, toileting, OR
- Require hands-on assistance with at least one of the above and unable to self-administer medication, OR
- Due to cognitive impairment, requires presence of another person on 24-hour-a-day basis for purpose of supervision
- Be able to be maintained safely and cost-effectively in own home
- Have attending physician approve home care plan
- Agree to total plan of care

individuals requesting help for themselves.
