The Chronic Disease Self-Management Program is a small group program that is designed to help adults gain confidence in their ability to manage symptoms and understand how their health problems affect their lives. CDSMP was developed by researchers at Stanford University and has been proven to be effective in improving health outcomes. Topics covered during the six week program include: 1) Techniques to deal with problems such as fatigue, frustration, pain and isolation 2) Appropriate exercise for maintaining and improving strength, endurance and flexibility 3) Appropriate use of medications 4) Communicating effectively with family, friends and health professionals 5) Nutrition 6) Decision making 7) How to evaluate new treatments The workshop will be held for six weeks every Monday from 9:30 am- 12:00 pm beginning on 10/16/17 and ending 11/20/17.
